Wear a shirt. The care team does the rest. Why simplicity is the hardest engineering problem in remote monitoring.

There's a mistake the wearable industry keeps making. It treats new sensors as the product. A new PPG channel, a new accelerometer, a new SpO₂ estimator — as if the way to win in remote monitoring is to add another number to the dashboard.

The number of sensors was never the problem. The problem is the distance between the signal and the person who needs to act on it.

eShirt's design philosophy is the opposite of feature accumulation. We start with the patient's day and work backwards. What does a 92-year-old man in Arkansas actually need to do to make continuous monitoring work? Ideally, nothing. He puts on a shirt.

Simplicity is an engineering discipline

Building a garment that captures clinical-grade cardiac, respiratory, temperature, posture, and activity signals continuously — through a shirt, wirelessly, without a strap, without an obvious device, without a daily calibration ritual — is dramatically harder than building a chest belt that does the same thing. The engineering complexity moves inward, so the wearer's complexity moves toward zero.

That trade-off is deliberate. The bar for a clinical monitoring device isn't just what it can measure. It's what the wearer will still be doing with it six months from now.

Simple for the wearer. Serious for the clinician.

On the wearer's side: put the shirt on. On the clinician's side: continuous signals, clinician-controlled alert thresholds, and a picture of physiologic status that persists through the hours between visits. That asymmetry — trivial to wear, serious to read — is the entire product thesis.
