Phase 1 — Pilot (3-6 months)
Objectives

Scope
- Single VAMC or VISN subgroup
- Heart failure patient population (n = 30-50 veteran participants)
- Dedicated clinical champion (cardiologist or HF specialist)
- IT support for EHR integration

- Device adherence > 80%
- Data quality > 95%
- Clinician satisfaction > 4/5
- No major safety events
- Clear integration pathway documented
Phase 2 — VISN-Level Rollout (6-12 months post-pilot)
- Expansion to 2-3 VAMCs within the pilot VISN
- Documented training protocols
- Integration optimized based on pilot learnings
- Pilot outcomes published in federal medical literature
Phase 3 — Enterprise (12+ months)
- National deployment decision
- VHA policy and directive updates
- Workforce training program
- Ongoing monitoring and optimization
